LEADERSHIP IN CATASTROPHIC INCIDENT PREVENTION: USING ORGANIZATIONAL DATA TO ASSESS AND RESPOND TO HIGH RELIABILITY LOSS PREVENTION PRECURSORS

Catastrophic Occupational & Process Safety Incidents usually occur after a series of early warning signals,often subtle,are overlooked.Some organizations have been very successful in avoiding catastrophic events,even in environments with high risk factors and complex operations.Known as Highly Reliable Organizations(HROs),these operations have developed cultures that embrace the concepts of Anticipation,Inquiry,Execution,and Resilience-leading to significant improvement in the detection of and response to early warning signals.Contemporary research has identified methods,which allow for identification of incident precursors to Serious & Catastrophic Potential Events,along with diagnostic tools to assess the maturity of an organization's”High Reliability”.These tools provide powerful insights,and allow leaders to develop a comprehensive approach to catastrophic incident prevention through the effective use of Organizational Data.
