All registrars and residents learn on their principle AO courses to apply external fixation to temporise an injury by bridging the zone of fixation as well as the zone of injury. While in an ideal world this would always occur, this is farfrom an ideal world, and often surgeons are left with no option other than to apply a fixator within the zone of fixation due to the anatomy of the fracture and patient. This can sometimes give the surgeon offering definitive fixation a tricky choice between overlapping the internal fixation with pin sites or using definitive external fixation.
