There has been a resurgenceof interest in the ankle syndesmo-sis, with recent papers looking ataccuracy of reduction, functionalrestriction, and range of motionfollowing syndesmosis injuries. Thisinterest has paralleled the develop-ment of the TightRope syndesmosisdevice (Arthrex, Naples, Florida),which allows a 'flexible' fixation ofthe syndesmosis using an endobutton and a knot through traditional drill holes. Although there is muchlow-quality evidence making theargument that this is a reasonableapproach, there is little in the wayof high-quality evidence comparingTightRope and traditional screwtreatment.
