Reverse BBRVT in a structurally normal heart

We present a case of bundle-branch reentry ventricular tachycardia with RBBB morphology (reverse BBRVT) in a patient with preexisting right bundle-branch block and without apparent structural heart disease.The diagnosis of BBRVT may be very challenging due to the similar wide QRS morphology during tachycardia and in sinus rhythm, rather suggesting a supraventricular origin. BBRVT should be suspected even in the absence of structural heart disease, but especially in patients with extensive underlying His-Purkinje conduction disorders. The aetiology, mechanism, diagnostic issues and treatment are briefly reviewed.
