Acquired hemophilia in a nonagenarian: A case report

Herein is reported a case of acquired hemophilia (AHA) in a 91-year-old woman with medical record of arterial hypertension, paroxysmal atrial fibrillation treated with warfarin, mitral valve insufficiency, and aortic valve sclerosis. She was admitted for 3 weeks of functional, clinical, and cognitive decline. One month before admission, she had fallen at home and developed a bilateral pectoral hematoma. Test results were hemoglobin 5.8 g/dL, normal platelet count, prolongation of prothrombin time (PT) 62.6% (normal 80-120%), and prolongation of activated partial thromboplastin time (aPTT) 50.2 seconds (normal 23-39 seconds). After admission, she developed large spontaneous hematomas on the left arm, right leg, and lower back associated with persistent anemia despite blood transfusions.
