A 63-year-old man presented 9 months after surgery for a fourth-ventricle ependymoma (figure A) with progressively blurred vision because of oscillation of the eyes. On physical examination he had a vertical pendular and gaze-evoked nystagmus (see webvideo 1). There was also a subtle palatal myoclonus (see webvideo 2). MRI of the head showed an asymmetrical enlargement of the inferior olivary nuclei with high signal intensity (figure B).
