Discussion: Impact and outcome of human acellular dermal matrix size for immediate and two-stage breast reconstruction

Multiple articles published previously have confirmed the beneficial effects of using human acellular dermal matrix in the setting of prosthetic breast reconstruction. These benefits include more precise control of the inframam-mary fold and better preservation of implant location, coverage of the lower pole of the implant as a pectoralis major muscle "extension," instillation of additional volume at the time of expander placement, maximal use of mastectomy skin flaps, reduction in the overall number of expansions, and decreased incidence of capsular contrac-ture. As a result, reconstructive outcomes have generally improved and become more predictable and reproducible. In addition, recent studies and systematic reviews have shown that complications using human acellular dermal matrix are comparable to those of non-human acellular dermal matrix breast reconstructions.
