A male infant aged 20 months (born prematurely at 27 weeks' gestation) with chronic lung disease (CLD) and on home oxygen presented with a 24-h history of respiratory distress.He was tachypnoeic with a respiratory rate of 60/ min. He was managed with continuous nebulised salbutamol, intravenous hydrocortisone and salbu-tamol but did not respond with oxygen saturations (SaO_2) persisting between 60% and 70%. He was therefore intubated and commenced on mechanical ventilation. He was transferred to the paedia-tric intensive care unit (PICU) for continuing management.
