Surgical reconstruction of postlaminectomy cervical kyphosis

The potential destabilizing effect of posterior decompressive procedures on the cervical spine has been well recognized in the literature. The incidence of progressive kyphotic deformity following cervical laminectomy may vary with certain patient characteristics, underlying diagnoses, and operative techniques. Measures to avoid postlaminectomy kyphosis have therefore been debated. Furthermore, the clinical impact of such postoperative deformity has recently been questioned. Treatment options range from observation to surgical reconstruction and arthrodesis, depending on the severity of the deformity and the degree of neurologic compromise. Recent literature has examined the frequency of postlaminectomy cervical kyphosis, the differing effects laminectomy and laminoplasty may have on cervical spine stability, and the indications and management options available for reconstruction of the kyphotic cervical spine following laminectomy. < copyright > 2002 Lippincott Williams & Wilkins, Inc.
