Wales progresses with e-discharge

COMMUNITY pharmacists in Wales could be sent tailor-made electronic discharge forms for patients. A current pilot only involves GPs, but the system could be applied to community pharmacists in the future. Pharmacists have been asked what information they would need in a hospital discharge form, in a survey conducted by the NHS Wales Informatics Service and Cardiff University.
