Openness and transparency

MOST people who are given the wrong medicine, or the right medicine at the wrong time or in the wrong dose, would like to be made aware of it - particularly if the mistake had led to them suffering a side effect or not receiving effective treatment.It is well documented that medication errors are commonplace even though they are not always picked up. By virtue of their training, pharmacists are good at letting patients know when things go seriously awry - but if they think no harm will befall the patient or the problems so minor as to be not worth mentioning, might they not be reluctant to say anything? That would be human nature.
