The current legal position as regards medication errors in accordance with the Medicines Act 1968 can be simply put as follows: if you make an error and supply an incorrect drug, you are a bad person and will be punished. This is normally referred to as a punitive culture and, historically, is the way laws and regulations are enforced. This may be the way society has preferred to develop in order to keep us all on the straight and narrow but in safety critical industries there is overwhelming evidence that this approach does not work.
