Pharmacists are invaluable when it comes to medicines reconciliation, the authors of a study have said, after finding that over one-third of patients have medication errors that occur when they are admitted to hospital.The study (Journal of General Internal Medicine 2010;25:441) compared pharmacist and hospital doctor medication histories with the drugs prescribed on admission at one Chicago hospital over 14 months.
