NPSA chief: report errors despite prosecution fear

Pharmacists should-continue to report medication errors even if they fear prosecution for a dispensing error, said Martin Fletcher, chief executive of the National Patient Safety Agency. Speaking at a press conference in London last week, Mr Fletcher added that, although there should be accountability for an incident, it is important that there is an open and fair culture with regards to the reporting process. David Cousins, the NPSA's head of safer medication and medical specialties, said "reporting is the safest option".
