Fatal overdose after prescribing error was manslaughter, jury rules

A jury has ruled that the death of a woman caused by an overdose of idarubicin after she was prescribed four times the amount she needed was manslaughter by gross neglect, returning a verdict of unlawful killing. Anna McKenna died in April 2006 after she was prescribed 60mg of the drug daily for four days instead of 60mg over four days. The mistake was not picked up by the pharmacy department at Frenchay Hospital in Bristol, where Mrs McKenna was being treated for multiple myeloma.
