In defence of his company's "tablet within a capsule"omeprazole,Mike Isaacson of Dexcel Ltd appears to have missed the point (PJ,26 May,p610).Having detected a potential problem with patient compliance due to a difficulty in swallowing,the doctor had the foresight to prescribe a capsule that could be opened and the contents sprinkled on yoghurt.In prescribing omeprazole generically,again doing what he or she thought was right,the doctor had unwittingly entered the patient into a lottery,where chance would determine whether she received the capsule containing enteric-coated granules,as intended,or containing a tablet.
