2009 is the first time that the entire process of nominations and voting for paper of the year has been open to anyone with access to the internet. The results of the 3521 votes cast online highlight research that struck a chord with readers, and excited them about new directions in practice and investigation. Surprisingly, the big health story of 2009, pandemic Influenza AH1N1, was not even nominated. Among nominations, studies dealing with clinical medicine tended to receive more votes than those addressing public health or investigating the scientific basis of disease.
