Mixed messages on systemic therapies for diabetic retinopathy.

Faramarz Ismail-Beigi and colleagues report, in a subanalysis of the ACCORD trial, that rates of the prespecified primary composite microvascular endpoint (dialysis or renal transplantation, high serum creatinine, retinal photocoagulation, or vitrectomy) and a secondary microvascular endpoint (primary endpoint plus peripheral neuropathy) were not different between intensive and standard glycaemic control. Although albuminuria was significantly decreased by intensive glycaemic control, doubling of serum creatinine concentrations or decreases in estimated glomerular filtration rate of more than 20 mL/min occurred more frequently.
