A C Keech and colleagues1 are to be commended on their monumental FIELD study, but I write to express amazement that they were able to find nearly 10000 patients with diabetes who were "without requiring lipid-modifying treatment at study entry". I very rarely see patients with diabetes who are not on a statin, and, given that at entry to the study more than 20% of the participants already had cardiovascular disease, it is little short of incredible that so many patients seem to have been denied standard treatment.
