Intensive insulin therapy in patients with type 2 diabetes.

Jianping Weng and colleagues are to be commended for building a concept that could change clinical practice. However, their conclusions need some clarification. First, we would like to propose an alternative mechanism for the larger remission rates seen after a short course of intensive insulin therapy than after a short course of oral hypoglycaemic agents. Confrontation with intensive insulin therapy is likely to induce a larger disease awareness and readiness to change in people diagnosed with an dften asymptomatic disease than are oral agents. This awareness could translate into better adherence to diet and exercise recommendations. If so, one would expect more weight loss after 1 year in the groups treated with a short course of intensive insulin. Was change in bodyweight at 1 year different in the three treatment groups?
