In August, 2005, a 31-year-old man presented to the emergency department of the Siena General Hospital with fatigue, dyspnoea, and chest pain. He had no previous medical history but admitted longstanding cocaine misuse,The patient's blood pressure was 130/80 mm Hg, and his serum troponin I concentration was 3mug/L; the urine toxicology screen was negative.
