Bladder 'geodes:' An unusual manifestation of cystolithiasis

A 71-year-old man with noninsulin dependent diabetes presented to the urology clinic with a history of irritative and obstructive voiding symptoms. On digital rectal examination the prostate was only slightly enlarged and prostate specific antigen was normal at 0.78 ng/ml. Urinalysis showed trace blood, urine culture was sterile and pH was 5.5. Post-void residual urine volume on BladderScan? was 0 ml and upper tract imaging studies were unremarkable. Before urological consultation the patient had received standard medical therapy for presumed benign prostatic hyperplasia but symptoms failed to improve.
