Despite the high NPV found in low risk patients, a 16.7% pT3 stage was still reported, which confirms that tumor burden is not well evaluated by biopsies in this group of patients.1 Tumor burden strongly affects MRI interpretation with regard to tumor visibility, tumor size and tumor contact with the capsule. Inapparent tumors and small visible tumors with a tumor contact with the capsule of less than 5 mm may represent the only true low risk population.
