Editorial Comment: Because the use of magnetic resonance imaging (MRI) before biopsy is becoming an area of investigation for a number of institutions, the optimal method by which biopsy is targeted to an MRI abnormal region will become a critical question for the urologist. In this series my group undertook a prospective trial to determine if software coregistration of ultrasound to MRI (also call MR-ultrasound fusion biopsy) allows more accurate biopsy of the MRI abnormal region than simple visual estimation of the lesion location on transrectal ultrasound guided biopsy. The study was likely under powered but showed a trend toward increased detection of high grade disease in the fusion guided biopsy, and demonstrated the fusion biopsy to be more often informative of a histo-logical abnormality explaining the MRI abnormal region.
