High grade prostatic intraepithelial neoplasia in prostate needle biopsy

Prostatic intraepithelial neoplasia (PIN) is the diagnostic term for premalignant proliferation of atypical epithelial cells within preexisting prostatic ducts and acini.1'2 High grade PIN is the most likely precursor proliferation for many moderately to poorly differentiated (Gleason score 5-10) carcinomas that arise in the peripheral zone. PIN is categorized as low grade and high grade. It is not necessary to diagnose low grade PIN since it does not indicate an increased risk of subsequent detection of carcinoma on needle biopsy.
