Atypical adenomatous hyperplasia (AAH), also known as adenosis, is a small glandular proliferation that may be misdiagnosed as adenocarcinoma, particularly on needle biopsy.1"3 AAH is invariably an incidental histological finding, and there are no clinical findings specific for it. Mean patient age at diagnosis is 64 to 70 years (range 45 to 83). It is not known whether AAH can elevate serum prostate specific antigen or produce a mass palpable on digital rectal examination, but this seems unlikely because most AAH foci are microscopic in size. The incidence of AAH depends on the type of tissue sample, with a wide range from less than 1% on needle biopsies to 22% to 60% in whole glands.
