Various reports have suggested that preoperative sunitinib causes only a small decrease in primary renal cell carcinoma tumor size. Rini et al (page 1548) from Cleveland, Ohio evaluated whether 50 mg sunitinib daily could be used to facilitate surgery in 58 patients. Computerized tomography was performed every week to determine resectability. Clear cell tumor size decreased 1.7 cm (-28%), whereas nonclear tumor size increased 0.1 cm (1.4%). Of the patients 45% were able to undergo nephrectomy. Preoperative sunitinib, which was withheld 7 days before surgery, resulted in a maximum reduction in tumor size after 2 cycles, although patients received 4 cycles.
