Nocturia: escaping the 'prostate-centric' approach.

Nocturia in particular and lower urinary tract symptoms in general are slowly escaping the "prostate centric" view, which formerly condemned men to undergo removal of the prostate with little thought regarding other potential causes of their symptoms. The guidelines and recommendations published in 2004 by the British Association of Uro-logical Surgeons, in 2009 by the International Consultation on Urological Diseases (ICUD),1 and in 2010 by the European Association of Urology and the United Kingdom National Institute for Health and Clinical Excellence, were each entitled "The Management of Lower Urinary Tract Symptoms in Men." Previously such important works would have used the misplaced abbreviation "BPH" (benign prostatic hyperplasia). However, the International Continence Society (ICS)2 and ICUD1 underlined the importance of the use of the term "benign prostatic obstruction" to emphasize the need to provide evidence that the prostate is causing obstruction and is not merely enlarged ("BPE") or showing his-tological change ("BPH"). Nocturia, one of the most bothersome of lower urinary tract symptoms, exemplifies the need to move away from the misuse of the term BPH. The authors of 2 articles in this issue of The Journal have addressed these concerns with intellectual rigor.
