We read the article "Outcome of Descemet's Stripping Automated Endo-thelial Keratoplasty (DSAEK) in Patients With an Anterior Chamber Intraocular Lens" with interest and would like to commend the authors on their successful results in these challenging cases.1 We wish to highlight these difficulties with reference to Descemet's stripping endo-thelial keratoplasty (DSEK) with manually prepared donor lenticules in patients who have undergone previous penetrating keratoplasty.
