We read with great interest the remarkable article by Boyd et al (1) in which they report a positive association between a higher fluid balance and higher central venous pressure with increased mortality in patients with septic shock. Although their findings are in accordance with the current literature, we believe that many questions remain on this issue.First, should we be surprised that the association between higher central venous pressure and positive fluid balance is weak being statistically significant at 12 hrs but not after 4 days? Not at all.
