Having the habit to think outside of the box, as attested by my contribution to the description of ventilator-induced lung injury (1) (one of the themes covered in the October supplement issue of Critical Care Medicine, which is entitled, "Thinking outside the box"), I was particularly pleased to read in another paper from the same issue that Dr. Vincent (2) recently came to the very same conclusions that I reached several years ago (3-5) on the limited value of randomized trials in critically ill patients.
