Haemorrhagic herpes zoster associated with clopidogrel treatment.

Anticoagulant and antiplatelet treatment may render patients liable to bleeding from various parts of the body. Inflammatory skin disorders are also potential sites of haemorrhage, leading to modifications of the classic clinical picture of the dermatoses. We report a case ofhaemorrhagic herpes zoster in a middle-aged woman undergoing treatment with clopidogrel, a new oral antiplatelet agent that is structurally a thienopyridine derivative and acts by irreversible inhibition of platelet aggregation.A 49-year-old woman presented with a history of acute onset of a blistering rash containing bloody fluid over her left arm, associated with severe burning sensation and lancinating pain that had started 2 days previously. There was no other evidence of external bleeding. The patient's medical history included chickenpox early in her childhood, and at the time of presentation, she was undergoing treatment with isosorbide mononitrate, enalapril and clopidogrel for ischaemic heart disease.
