There have heen many facelift techniques reported in the literature proving there is no recognizable standard procedure, Since SMAS anatomy information has been studied in detail, various facelift procedures such as the extended SMAS and deep-plane facelift have been heavily developed, However, there has been controversy over the risks and effectiveness.It is accepted that the facelift operation is more difficult for Asians than for Caucasians due to the facial bone structure and the characteristics of soft tissue.Because of this, more complicated techniques are required to obtain better results.As a new concept for Orientals, the author divides the cheek area into two and uses a different procedure in each zone.Initially, the author performs SMAS ectomy in the lateral zone and SMAS plication and malar fat pad suspension with APTOS threads in the medial zone, Next, the skin is re-draped posterolaterally, anchored by retaining ligament sutures thus diminishing the nasolabial fold, and finally, redundant skin is excised.Although the procedure is complicated and time-consuming, it results in an optimal aesthetic rejuvenation while preventing major complications.
