We appreciate that these are evidence-based proposals to prompt discussion and development of formal guidelines for this spectrum of disorders that require multidis-ciplinary pre- and perioperative input. However, we wish to draw attention to a few points of interest that have raised discussion at a district general hospital level.Firstly, the advice published has important implications for specialist centres. Are they logistically able to accommodate this new cohort of patients? Regional communication with these centres is vital to clarify their capacity for accepting these groups via formalised agreement between both hospitals.
