Introduction

Pharmacy practice has changed throughout the years evolving from the traditional dispensing only duties to now being more clinically based, due to Medication Therapy Management (MTM). MTM is a common term used and performed primarily by pharmacist, identifying a wide range of clinical health care services. As defined in a consensus definition adopted by the pharmacy profession in 2004, MTM is a service or group of services that optimize therapeutic outcomes for individual patients. MTM services include medication therapy reviews, pharmacotherapy consults, anticoagulation management, immunizations, health and wellness programs, and many other clinical services.
