A second look at the fresh frozen plasma: packed red blood cell ratio in massive transfusion protocols.

We read with great interest the article presented by Dr Riskin and colleagues regarding the implementation of a Massive Transfusion Protocol (MTP).1 In this article, the authors critically reviewed mortality differences before and after the implementation of an MTP at their institution.Interestingly, although one of the goals of the MTP was to increase the fresh frozen plasma to packed red blood cell ratio (FFP:PRBC) to a target of 1:1.5, when the years prior to implementation were compared to the two consecutive years thereafter, the FFP:PRBC ratio, evaluated at 24 hours, did not change (1:1.8, in both periods), but mortality decreased from 45% to 19% (p = 0.02).
