Acardiac twins have been named according to the main anatomical abnormality (holoacardius or pseudoacardius), the apparent patho-physiological basis (TRAP sequence) or the total dependence on the normal co-twin (chorioangiopagus parasiticus) (Tan and Sepulveda 2003). This malformation is a rare occurrence, with an incidence of 1 per 18,000-34,600 deliveries. It occurs in about 1 in 100 of monozygotic twins and 1 in 30 monozygotic triplets (Gillim and Hendricks 1953; Chanoufi et al. 2004). We present a case of acardiacus acephalus type twin pregnancy complicated with polyhydramnios and pre-term birth.
