A 71 -year-old woman had menopause at 52 years of age and had never had hormone therapy. She presented with abnormal dark red vaginal discharge and was therefore referred to the obstetrics and gynecology department. The result of the Pap smear was atrophic change, and further transvaginal ultrasonography showed a right adnexal mass. Pelvic examination showed atrophic and grossly normal cervix and vagina. Transvaginal ultrasonography showed a cystic tumor with central vegetations about 5 X 4 cm in size in the right adnexal area. Serum CA 125 and CA 199 were with normal range, but carcinoembryonic antigen level had increased to 20 ng/mL. An adnexal cystic tumor with suspicion of malignancy was considered.
