Perioperative hemodynamic disturbances are common in neuro-surgical procedures. In this regard, trigeminal cardiac reflex (TCR) has been reported to be one of the causes of these events. However, this reflex was rarely reported in patients undergoing awake craniotomy.1 A 50-year-old patient was admitted to our hospital with complaints of left-sided frontal headache. She was diagnosed as a case of left frontal space-occupying mass. As this tumor was near the eloquent area of brain (speech and motor cortex), awake left-sided frontal craniotomy was planned. The patient was nondiabetic and nonhypertensive. Airway assessment was normal.
