The Scottish Intercollegiate Guidelines Network (SIGN) has a long and very creditable history of producing high-quality clinical guidelines based on careful systematic review and expert overview. This guideline is no exception and represents a considerable amount of work. The document is well referenced, with over 160 references as well as the systematic review that has been used to inform the recommendations.
