Endoscopy or microsurgery: Is the never-ending debate concerning the choice of surgical strategy for colloid cysts of the third ventricle still a topical issue or has it been resolved?

The anatomical location of colloid cysts implanted in contact with the choroid plexus of the roof of the third ventricle accounts for the difficulties of the surgical approach and the debate concerning the choice of the best surgical access to these lesions. Taking advantage of dilation of the lateral ventricles caused by obstruction of the foramina of Monro has been considered for a long time to be the best strategy, as this dilation of the ventricles already provides part of the surgical access, and neurosurgeons are familiar with this approach. However, this approach requires opening of the frontal cortex and the creation of a sufficiently large well to allow the use of instruments at the base of the cavity in contact with the foramina of Monro, using an operating microscope.
