F-18 fluorodeoxyglucose (FDG) PET/CT contributes to the diagnosis of malignancy and the detection of recurrence of malignancy after therapy. However, FDG uptake in physiologic and benign processes is well documented. Misinterpretation of FDG uptake in brown fat may result in an erroneous definition of PET findings as malignant. We present the case of FDG uptake in parahepatic brown fat, which mimicked peritoneal carcinomatosis in a malignant ovarian germ cell tumor. Repeat PET/CT scan after the control of the patient's environmental temperature to 75 deg C showed resolution of the parahepatic brown fat uptake.
