It must be acknowledged that the level of quality in our health care system is insufficient today despite the resources allocated by our government (Institute of Medicine [IOM], 2004). We have forge populations of patients whose needs have not been adequately met by traditional models or systems of health care.The complexity of health care in the 21st century demands, more than ever, a comprehensive approach to coordination of the many facets of care for safe and effective treatment.
