A 65-year-old man underwent a 18F-FDG positron emission tomography/computed tomography (PET/CT) scan for staging of lymphoma. 18F-FDG PET showed lymphomatous lung deposits and widespread nodal lytnphomatous deposits above and below the diaphragm. The configuration of the mediastinum was abnormal. Further, widespread deposits were noted in the presumed liver. However, combined PET/CT allowed diagnosis of situs inversus totalis with splenic lymphomatous infiltration and not liver as initially presumed on the PET only images.
