There were many reasons that I read with great interest the article by Murphy et al. on branched stent-graft repair of a chronic type A aortic dissection in a patient with multiple prior stemotomies in this issue of JEVT. First, though, I would like to congratulate the authors for finding the right strategy and technique to treat this particular patient. I totally agree that other options were inappropriate, which allowed the authors to go with this "special" option in a (hopefully) fully informed patient!
