Hypertrophic cardiomyopathy (HCM) is a monogenic cardiac disease with an autosomal dominant pattern of heredity. It has been defined morphologically by unexplained hypertrophy in the absence of hemodynamic stress, at the histo-logical level by myocyte disarray, fibrosis and abnormalities of intramyocardial small vessels. The majority of patients with obstructive HCM are asymptomatic, but some can present with heart failure or fatal arrhythmias. While the etiology of HCM has been studied extensively, the pathogenesis is not completely understood. There is extensive research on biomarkers and their potential use in identifying patients with the disease and in providing risk stratification. In recent studies, data about inflammatory, apoptotic, endothelial function, platelet function, prothrombotic, matrix metalloproteinases, growth factors and wall stress markers have been extensively published (1). Sato et al. (2) observed increased troponin concentrations in patients with HCM and suggested that the increase in troponin I (TnI) may be an indicator of disease progression. However, to date there is little data about the novel markers of myocardial ischemia and necrosis.
