Images in cardiology: congenitally corrected transposition of the great arteries with dextrocardia.

In a 67-year-old man admitted with dyspnea and impaired daily life activities, chest x-ray showed dextrocardia, cardiomegaly, and bilateral pleural effusion (Fig. 1). Transesophageal echocardiogram and computed tomography demonstrated congenitally corrected transposition of the great arteries with dextrocardia, and without any other cardiac anomalies (Fig. 2A, B). The echocardiogram also revealed severe systemic (right) ventricular dysfunction with ejection fraction 42%, marked systemic ventricular dilatation and hypertrophy, and moderate systemic (tricuspid, right) atrioventricular valve regurgitation. The patient underwent medical treatment with diuretics, an angiotensin receptor blocker, and a beta adrenergic receptor antagonist. His daily life activities improved thereafter.
