Cataract surgical problem

This patient is experiencing the classic problems associated with implantation of an oversized AC IOL. They include iris tuck, iris atrophy, ovalization of the pupil, ectropion pupillae, anterior synechiae, globe tenderness, and corneal decompensation. Although no mention was made of glaucomatous optic nerve changes, the IOP in the right eye is twice that in the left eye and will have to be monitored going forward. The classic starting point for sizing an AC IOL is the horizontal corneal WTW diameter + 1.0 mm. If after implantation, an IOL is found to be oversized or undersized, it should be exchanged immediately to prevent the problems that developed here.
