Commentary: Peripheral neuromodulation for pain.

The review by Bittar and Teddy1 presents the available data on the topic of implantation of peripheral nerve electrodes and pulse generators for the management of a potpourri of chronic pain conditions. The data demonstrate the absence of valid evidence (Class I) that would justify clinical use of this expensive technology. The authors have reviewed papers that are best described as case reports, or small case series. No case-control trial has ever been reported, let alone a randomised case-control trial. This is despite the widespread use of such devices in some centres, and the exorbitant financial expense involved.
