Because Patient A had chronic poly-dipsia and had no history of bipolar disorder, and no change in his schizophrenia symptoms was noted over the course of his treatment, topiramate was felt to be a salient factor in his improvement. The threshold effect noted in his response suggests that the effect of topiramate is not linear and that a minimum dose may be required to achieve efficacy. The efficacy of topiramate in this individual may be related to activity at AMPA/kainate receptors in the hypothalamus because they are implicated in the regulation of feeding behaviors.16'17 Alternately, Patient A's poly-dipsia may represent a form of addictive or compulsive behavior.
